Gisele Nelson - Director
As a Director of HCBS Services, Gisele's primary goal is to ensure all children with mental illness and/or medical fragile is linked to services and families are supported within the 5 boroughs. Gisele has been servicing the Human Services field for the past 15 years and board experience linking the under-serviced population. Gisele received her Master's degree in Social Work from Lehman College. After completing her education, she accepted a Director position at PostGraduate Center for Mental Health (PGCMH) and helped developed a Supportive Case Management (SCM) pilot program. The success of this program led to the creation of SCM programs statewide. During her tenure at PGCMH, she managed the development and administration of Adolescent Supportive Case Management, Children Intensive Case Management, City-Wide Health Homes, and Health Home Plus programs.
Luisa Taveras - Office Manager
I am compassionate about providing support to underserving children. I am a dedicated professional bilingual provider with 11 years of experience working with children and families struggling with mental health. I am committed to providing top quality care and personalized service to each child and family on my caseload. I work well on a team and utilize all appropriate resources when needed. I help their families understand their options concerning the specific situation they are dealing with at the time. I am a liaison between families and their treatment or care options. I also have extensive experience with creating and cultivating community partnerships. In the process of furthering my education, I will be obtaining my bachelor’s degree in healthcare administration in December 2025.
